Payment / Insurance Policy
Payment for services are the responsibility of the patient. All copays and deductibles are due at the time of the visit. Many patients have health insurance and rely on their coverage to pay the major portion of their health care costs. It is important for you to understand that health insurance is a contract between you “the patient” and the insurance company, we are not a party of that contract. Therefore it is a good idea for you to become familiar with your own insurance coverage. It is important to remember that whatever portion of services not paid by your insurance is your responsibility. Questions on insurance matters should be discussed with the office manager, who is experienced in handling insurance claims.
Referrals
Unless your insurance allows you to go directly to a specialist, you must obtain a referral from your primary care physician, prior to receiving treatment in our office. It is your responsibility to coordinate in a timely fashion, so that you can bring the referral at the time of the visit.

Podiatry Home Care
Podiatry house calls are for all medically homebound patients or patients that have extreme difficulty with transportation to and from the office.
